Appointments :
You will need to obtain a referral letter from your General Practitioner or Specialist for your initial consultation.
You should also bring the following:
- Medicare card
- Pension, Department of Veterans’ Affairs (DVA) or Concession card
- Copies of any relevant blood test results, x-rays or scans
- Private health insurance fund details.
Payment for your consultation is required on the day by cash, EFTPOS, credit card or money order. Medicare provides a rebate for part of Mr. Chang's fee. Please ask for further details from our reception staff.
